Role Overview
The Care Coordinator plays a central role in the day-to-day coordination and delivery of safe, reliable and high-quality homecare services.
The successful candidate will be responsible for coordinating client schedules, supporting Healthcare Assistants, managing changes to care delivery, communicating with clients and families and ensuring that services are delivered in line with agreed care plans, company procedures and contractual requirements.
This is a fast-paced role requiring excellent organisation, communication and problem-solving skills, along with the ability to manage competing priorities and respond effectively to changing service needs.

Key Responsibilities
Care Coordination & Scheduling
Coordinate and maintain client care schedules across Wexford and Gorey.
Match Healthcare Assistants to clients based on availability, location, skills and client requirements.
Manage rota changes, annual leave, sickness and short-notice absences.
Ensure all scheduled client visits are appropriately covered.
Respond promptly to gaps or changes in service delivery.
Work closely with the wider operations team to maintain continuity of care.
Monitor schedules to support efficient use of staff and minimise unnecessary travel.
Client & Family Communication
Act as a key point of contact for clients and their families regarding scheduled care services.
Communicate changes to care schedules promptly and professionally.
Respond to queries, concerns and service issues in a timely manner.
Escalate complaints, safeguarding concerns or significant changes in a client's condition appropriately.
Support positive and professional relationships between clients, families, Healthcare Assistants and the office team.
Healthcare Assistant Support
Provide day-to-day support and guidance to Healthcare Assistants.
Communicate rota updates, service changes and client information clearly.
Monitor attendance, punctuality and completion of scheduled visits.
Escalate repeated attendance, performance or conduct concerns to the appropriate manager.
Support Healthcare Assistants to work safely and within their role and scope of practice.
Assist with resolving operational issues that arise during care delivery.
Compliance & Service Governance
Ensure accurate records are maintained in relation to schedules, client communications and service changes.
Support compliance with Right at Home policies and procedures, HSE contractual requirements and relevant legislation.
Maintain confidentiality and handle personal information in accordance with GDPR and data protection requirements.
Ensure incidents, complaints, missed visits and service disruptions are recorded and escalated appropriately.
Work closely with the compliance and management teams to support safe and consistent service delivery.
Safeguarding & Risk Management
Recognise and appropriately escalate safeguarding concerns.
Follow company and HSE safeguarding procedures where concerns are identified.
Report incidents, accidents and risks promptly.
Support immediate risk management where concerns arise in relation to clients, staff or service delivery.
Escalate serious or urgent concerns to senior management or emergency services where appropriate.
Communication & Teamwork
Work closely with Business Managers, Supervisors, Healthcare Assistants, Recruitment, Compliance and other support functions.
Liaise professionally with clients, family members, Public Health Nurses and other healthcare professionals where required.
Participate in operational meetings and provide updates on scheduling, staffing and service delivery.
Ensure clear handover of outstanding issues to relevant colleagues.
Maintain professional and respectful communication at all times.
Health & Safety
Promote safe working practices across the service.
Respond appropriately to health and safety concerns raised by clients or Healthcare Assistants.
Support safe lone-working practices.
Report accidents, incidents, hazards and near misses in accordance with company procedures.
Work in accordance with the Safety, Health and Welfare at Work Act 2005 and relevant company policies.

Performance within the role may include:
Percentage of scheduled visits successfully covered.
Reduction in missed or disrupted client visits.
Timely management of rota changes and absences.
Response times to client and Healthcare Assistant queries.
Accuracy of scheduling and service records.

Job Specification
Essential Criteria
Qualifications
QQI Level 5 qualification in Healthcare Support, Community Care, Social Care or a recognised equivalent is desirable.
Relevant safeguarding training.
Manual Handling certification desirable.
Experience
Previous experience within homecare, healthcare or a similar service environment.
Experience coordinating staff, schedules or rotas is highly desirable.
Experience dealing directly with clients, families and frontline employees.
Previous experience within an HSE-funded homecare environment would be an advantage.
Knowledge

Working Conditions
Full-time permanent position.
40 hours per week.
Salary of €30,000–€33,000 per annum, depending on experience.
Based in the Wexford / Gorey area.
Primarily Monday to Friday, with flexibility required depending on operational needs.
The position involves regular communication with Healthcare Assistants, clients, families and the wider management team.
Occasional support outside normal office hours may be required depending on service needs.
Accountability
The Care Coordinator is responsible for supporting the safe, effective and reliable delivery of homecare services across Wexford and Gorey.
The role is accountable for maintaining accurate schedules, supporting Healthcare Assistants, responding to service changes and ensuring that client care is coordinated in accordance with Right at Home standards, our values of Accountable, Authentic, Personable and Thrive, relevant HSE requirements, company policies and applicable legislation.
